Glucophage or Glucophage XR (metformin), in the class biguanides, is the recommended first-line oral treatment for type 2 diabetes by the American Diabetes Association (ADA). Metformin does not cause weight gain or elevate insulin levels. As more data become available, evidence is mounting that thiazolidinediones may provide direct benefits to the heart and large blood vessels. They may prevent progression to diabetes in prediabetic individuals at high risk by reducing inflammation and by decreasing clotting factors.
Type 2 diabetes and prediabetes can be prevented with lifestyle changes, for example, eating a healthy diet, getting more physical activity, reducing stress, quit smoking, and reducing or managing blood pressure and cholesterol, and managing any other health conditions or risk factors that you may have for developing type 2 diabetes.
Yet understanding what and how much to eat can be a challenge. A registered dietitian can help you create a meal plan that fits your health goals, food preferences and lifestyle. This will likely include carbohydrate counting, especially if you have type 1 diabetes.
Eventually, however, many people with type 2 diabetes find that despite their best efforts, weight control, exercise and diet aren’t enough to keep their blood glucose in a healthy range. One theory is that some people’s insulin-producing cells just get tired out from having to produce more and more insulin because their cells are resistant to the effects of insulin.

Biguanides is a class of drugs that decrease the amount of glucose produced by the liver, and have been used for many years in Europe and Canada. In 1994, the FDA approved the use of the biguanide called metformin (Glucophage) for the treatment of type 2 diabetes.
