Type 1 diabetes (also called “juvenile”/young diabetes) is different than type 2 diabetes because it occurs when insulin-producing cells of the pancreas get destroyed by the immune system, therefore no insulin is produced and blood sugar levels go unmanaged. Type 1 diabetes tends to develop at a younger age, usually before someone turns 20 years old. Diabetes-related foot problems (such as diabetic foot ulcers) may occur, and can be difficult to treat, occasionally requiring amputation. Additionally, proximal diabetic neuropathy causes painful muscle atrophy and weakness.

There is no single dietary pattern that is best for all people with diabetes. For overweight people with type 2 diabetes, any diet that the person will adhere to and achieve weight loss on is effective.[75][76]

Rosiglitazone (Avandia) has been associated with an increased risk of heart attack and stroke, and experts have debated the severity of these concerns. On September 23, 2010, the U.S. FDA announced significant restrictions on rosiglitazone (Avandia) for those with type 2 diabetes. These new restrictions responded to data suggesting elevated risk of cardiovascular events (for example, heart attack and stroke) in patients treated with rosiglitazone. Men, women, and children can develop diabetes, but the disease can present problems unique to women. A 2007 study found that between 1971 and 2000, death rates for men with diabetes declined, but death rates for women did not.

Interventions to improve adherence can achieve reductions in diabetes morbidity and mortality, as well as significant cost savings to the health care system.[75] Smartphone apps have been found to improve self-management and health outcomes in people with diabetes through functions such as specific reminder alarms,[76] while working with mental health professionals has also been found to help people with diabetes develop the skills to manage their medications and challenges of self-management effectively.[77]

Another critic of the ADA program is futurologist and transhumanist Ray Kurzweil, who with Terry Grossman co-authored Fantastic Voyage: Live Long Enough to Live Forever (published 2004). They describe the ADA guidelines as “completely ineffective”. Their observations are that the condition, particularly in its early stages, can be controlled through a diet that sharply reduces carbohydrate consumption. Their guidelines for patients with type 2 diabetes is a diet that includes a reduction of carbohydrates to one sixth of total caloric intake and elimination of high glycemic load carbohydrates. As someone who diagnosed with diabetes but who no longer has symptoms of the disease, Kurzweil is a firm advocate of this approach. However, Kurzweil’s prescription changed somewhat between his 1993 book The 10% Solution for a Healthy Life in which he recommended that only 10% of calories should come from fat and Fantastic Voyage which recommends 25%.

In mice, researchers have been able to reverse some symptoms of diabetes and restore pancreas functions by putting them on a version of the fasting-mimicking diet. (9b) This is a diet that involves severe caloric restriction for five days out of the month. It follows the same principle as fasting by temporarily depriving the body of food to take advantage of health benefits like increased fat burning and reduced inflammation.

Those who’d had been diagnosed with diabetes more recently saw greater blood sugar improvements on the program. Ditto for those who lost the most amount of weight and/or made the greatest progress in raising their fitness level. Gestational diabetes mellitus (GDM) resembles type 2 DM in several respects, involving a combination of relatively inadequate insulin secretion and responsiveness. It occurs in about 2–10% of all pregnancies and may improve or disappear after delivery.[46] However, after pregnancy approximately 5–10% of women with gestational diabetes are found to have diabetes mellitus, most commonly type 2.[46] Gestational diabetes is fully treatable, but requires careful medical supervision throughout the pregnancy. Management may include dietary changes, blood glucose monitoring, and in some cases, insulin may be required.

There are several great exercises for diabetes, including biking, running, swimming, walking, strength training, and the like. The Centers for Disease Control and Prevention (CDC) recommends getting at least 150 minutes of moderate-intensity aerobic activity per week — that’s five 30-minute workouts — or 75 minutes of vigorous-intensity aerobic activity per week.

Type 2 diabetes and prediabetes can be prevented with lifestyle changes, for example, eating a healthy diet, getting more physical activity, reducing stress, quit smoking, and reducing or managing blood pressure and cholesterol, and managing any other health conditions or risk factors that you may have for developing type 2 diabetes.

The risk of developing type 2 diabetes increases with age, and the disease becomes more common in people over age 45. As stated above, anyone 45 years of age or older who is overweight or obese and that has at least one other risk factor for diabetes should get tested for diabetes.

Infections: Certain infections — such as frequent yeast infections of the genitals, dental infections, skin infections, and frequent urinary tract infections — may result from suppression of the immune system by diabetes and by the presence of glucose in the tissues, which allows bacteria to grow. These infections can also be an indicator of poor blood sugar control in a person known to have diabetes.

While liver problems reported with these agents have been mild (and reversible with discontinuation of the drug), most health-care professionals follow the earlier recommendation monitor blood tests for potential liver injury (every two months or so) during the first year of therapy. In type 2 diabetes, the pancreas does not produce enough insulin to keep glucose levels normal (in type 1, the pancreas stops producing insulin altogether), and if I didn’t take action, I would be 36 per cent more likely to die early and could suffer bad sight, poor kidneys, heart failure and strokes. If you have type 1 diabetes, avoid vigorous physical activity when you have ketones in your blood or urine. Ketones are chemicals your body might make when your blood glucose level is too high, a condition called hyperglycemia, and your insulin level is too low. If you are physically active when you have ketones in your blood or urine, your blood glucose level may go even higher. Ask your health care team what level of ketones are dangerous for you and how to test for them. Ketones are uncommon in people with type 2 diabetes.

A good number of diabetics, however, have the illness but don’t know it for at least five years before diagnosis. This is crucial because over time, the insulin-producing cells in the pancreas decline in function. Often, by the time a patient is diagnosed, a critical number of cells have stopped producing insulin entirely. There is no way to reverse this. If your diabetes is diagnosed early in the disease process, however, aggressive management may help you prevent further loss of function in those cells. This means maintaining your fasting glucose levels below 100 mg/dl and your after-meal (two hours after) levels below 140 mg/dl. This is the same for morning and evening glucose levels.

An approach that has been popular with some people with type 1 diabetes mellitus since 2000 is known as DAFNE (Dose Adjustment for Normal Eating). This approach involves estimating the amount of carbohydrates in a meal and modifying the amount of insulin one injects accordingly. An equivalent approach has for people with type 2 diabetes mellitus is known as DESMOND, which stands for Diabetes Education and Self-Management for On-Going and Newly Diagnosed (diabetes). DAFNE has a newsletter and has received recommendation.[16]

If the A1C test isn’t available, or if you have certain conditions — such as if you’re pregnant or have an uncommon form of hemoglobin (known as a hemoglobin variant) — that can make the A1C test inaccurate, your doctor may use the following tests to diagnose diabetes:

In a three-month study, repaglinide (Prandin) dropped fasting blood glucose values by 61 mg/dL and post-meal blood glucose values by 100 mg/dL. Because repaglinide is short-acting and given before meals, it is particularly beneficial in lowering blood glucose after meals and does not tend to lower fasting glucose levels to the same degree. Prandin has been used in combination with other medications, such as metformin (Glucophage), with impressive results. In 83 patients with type 2 diabetes, addition of repaglinide to metformin significantly improved blood sugar control.

Exactly why this happens is uncertain, although it’s believed that genetic and environmental factors play a role in the development of type 2 diabetes. Being overweight is strongly linked to the development of type 2 diabetes, but not everyone with type 2 is overweight.

A healthy eating plan is an essential part of any diabetes treatment plan, but there is no one recommended “diabetic diet” for everyone. An individual nutrition plan will depend on many things, including underlying health and level of physical activity, the types of medication(s) being taken, and personal preference. Most people with type 2 diabetes find that having a fairly regular schedule for meals and snacks is helpful. Eating a variety of foods and watching portion sizes is also recommended.

Most kinds of physical activity can help you take care of your diabetes. Certain activities may be unsafe for some people, such as those with low vision or nerve damage to their feet. You may still feel hungry even after you’ve had something to eat. This is because your tissues aren’t getting enough energy from the food you’ve eaten. If your body is insulin resistant or if your body doesn’t produce enough insulin, the sugar from the food may be unable to enter your tissues to provide energy. This can cause your muscles and other tissues to raise the “hunger flag” in an attempt to get you to eat more food.

Foot damage. Nerve damage in the feet or poor blood flow to the feet increases the risk of various foot complications. Left untreated, cuts and blisters can develop serious infections, which often heal poorly. These infections may ultimately require toe, foot or leg amputation.

Experts recommend that everyone, people with diabetes, make at least half of grains consumed daily whole grains — so make sure some of the starches you choose to eat contain whole grains. There are some interesting developments in blood glucose monitoring including continuous glucose sensors. The new continuous glucose sensor systems involve an implantable cannula placed just under the skin in the abdomen or in the arm. This cannula allows for frequent sampling of blood glucose levels. Attached to this is a transmitter that sends the data to a device. This device has a visual screen that allows the wearer to see, not only the current glucose reading, but also the graphic trends. In some devices, the rate of change of blood sugar is also shown. There are alarms for low and high sugar levels. Certain models will alarm if the rate of change indicates the wearer is at risk for dropping or rising blood glucose too rapidly. One version is specifically designed to interface with their insulin pumps. In most cases the patient still must manually approve any insulin dose (the pump cannot blindly respond to the glucose information it receives, it can only give a calculated suggestion as to whether the wearer should give insulin, and if so, how much). However, in 2013 the US FDA approved the first artificial pancreas type device, meaning an implanted sensor and pump combination that stops insulin delivery when glucose levels reach a certain low point. All of these devices need to be correlated to fingersticks measurements for a few hours before they can function independently. The devices can then provide readings for 3 to 5 days.

A balanced, healthy diet doesn’t mean giving up your favorite foods or going on a starvation diet. But you’ll probably have to limit junk food and sweets and eat smaller portions of foods if you’re overweight.

More adults get type 1 diabetes than children. It’s called LADA (latent autoimmune diabetes in adults). Ten percent of people who have been diagnosed with type 2 actually have type 1. And the medicines for type 2 make type 1 worse! It’s a simple blood test, but very few doctors (and even some bad endocrinologists) actually order it. Gene therapy can be used to turn duodenum cells and duodenum adult stem cells into beta cells which produce insulin and amylin naturally. By delivering beta cell DNA to the intestine cells in the duodenum, a few intestine cells will turn into beta cells, and subsequently adult stem cells will develop into beta cells.
