For most people with type 2 diabetes, weight loss also can make it easier to control blood glucose and offers a host of other health benefits. If you need to lose weight, a diabetes diet provides a well-organized, nutritious way to reach your goal safely.

A healthy breakfast should include one or more servings of a high-fiber whole grain food, a good source of low-fat protein or dairy (such as 8 ounces of fat-free milk), and a serving of fruit. Work with a registered dietitian or diabetes educator to determine the best foods and portions for you.

If you have signs or symptoms of low blood sugar, drink or eat something that will quickly raise your blood sugar level — fruit juice, glucose tablets, hard candy, regular (not diet) soda or another source of sugar. Retest in 15 minutes to be sure your blood glucose levels have normalized.

Type 2 diabetes symptoms often develop over several years and can go on for a long time without being noticed (sometimes there aren’t any noticeable symptoms at all). Type 2 diabetes usually starts when you’re an adult, though more and more children, teens, and young adults are developing it. Because symptoms are hard to spot, it’s important to know the risk factors for type 2 diabetes and visit your doctor if you have any of them.

The American Diabetes Association recommend a pre-meal blood sugar level of 80-130 milligrams per deciliter. Around 1 to 2 hours after the beginning of a meal, blood sugar should be less than 180 milligrams per deciliter.

People with diabetes may have problems with their feet because of poor blood flow and nerve damage that can result from high blood glucose levels. To help prevent foot problems, you should wear comfortable, supportive shoes and take care of your feet before, during, and after physical activity.

An article summarizing the view of the American Diabetes Association[19] contains the statement: “Sucrose-containing foods can be substituted for other carbohydrates in the meal plan or, if added to the meal plan, covered with insulin or other glucose-lowering medications. Care should be taken to avoid excess energy intake.” Sucrose does not increase glycemia more than the same number of calories taken as starch.[citation needed] It is not recommended to use fructose as a sweetener.[citation needed] Benefits may be obtained by consumption of dietary fiber in conjunction with carbohydrate; as Francis (1987) points out, evidence suggests that carbohydrate consumed with dietary fiber will have a lower impact on glycemic rise than the same amount of carbohydrate consumed alone. Due to their high levels of dietary fibre, certain foods are particularly beneficial for people with diabetes such as legumes, nuts, fruits, and vegetables.[20]

When fasting blood glucose stays above 100mg/dl, but in the range of 100-126mg/dl, this is known as impaired fasting glucose (IFG). While patients with IFG or prediabetes do not have the diagnosis of diabetes, this condition carries with it its own risks and concerns, and is addressed elsewhere.

In addition to eating peppers fresh, they can be added to pastas, eggs, and stir-fries. Roasting is another delicious preparation. Simply place whole peppers on a broiling pan, broil on each side about 4 inches away from heat until slightly charred, then place peppers in a plastic bag. At the same time, a serving of this nonstarchy vegetable is super low in calories (7) and carbohydrate (1 gram). A ½-cup cooked serving contains just 22 calories and 4 grams of carb.

Fig. 8.2 outlines these options, as well as recommendations for further intensification, if needed, to achieve glycemic goals. If a patient is still above the A1C target on premixed insulin twice daily, consider switching to premixed analog insulin three times daily (70/30 aspart mix, 75/25 or 50/50 lispro mix). In general, three times daily premixed analog insulins have been found to be noninferior to basal-bolus regimens with similar rates of hypoglycemia (62). If a patient is still above the A1C target on basal insulin plus single injection of rapid-acting insulin before the largest meal, advance to a basal-bolus regimen with ≥2 injections of rapid-acting insulin before meals. Consider switching patients from one regimen to another (i.e., premixed analog insulin three times daily to basal-bolus regimen or vice-versa) if A1C targets are not being met and/or depending on other patient considerations (60,61). Metformin should be continued in patients on combination injectable insulin therapy, if not contraindicated and if tolerated, for further glycemic benefits.

The three main types of nutrients found in foods are carbohydrates (or carbs), proteins, and fats, which all provide energy in the form of calories. Supplements don’t cure diabetes, either. Some natural supplements may interact dangerously with your diabetes medication. Others have been shown to help improve your diabetes, but always check with your doctor before taking any supplement.
