Insufficient production of insulin (either absolutely or relative to the body’s needs), production of defective insulin (which is uncommon), or the inability of cells to use insulin properly and efficiently leads to hyperglycemia and diabetes.

Random (non-fasting) blood glucose level — diabetes is suspected if higher than 200 mg/dL (11.1 mmol/L) and accompanied by the classic symptoms of increased thirst, urination, fatigue, hunger and weight loss. This test must be confirmed with a fasting blood glucose test. In patients who do not have diabetes symptoms, and whose random serum glucose level is over 140 mg/dL, a FPG or HbA1c level should be measured.
