In November 2016, the FDA approved two different once-daily fixed-dual combination products containing basal insulin plus a GLP-1 receptor agonist: insulin glargine plus lixisenatide and insulin degludec plus liraglutide. Other options for treatment intensification include adding a single injection of rapid-acting insulin analog (lispro, aspart, or glulisine) before the largest meal or stopping the basal insulin and initiating a premixed (or biphasic) insulin (NPH/Regular 70/30, 70/30 aspart mix, 75/25 or 50/50 lispro mix) twice daily, usually before breakfast and before dinner. Each approach has its advantages and disadvantages. For example, providers may wish to consider regimen flexibility when devising a plan for the initiation and adjustment of insulin therapy in people with type 2 diabetes, with rapid-acting insulin offering greater flexibility in terms of meal planning than premixed insulin. Ginkgo, garlic, holy basil leaves, fenugreek seeds, ginseng, and hawthorn are other herbals that have been promoted by some as remedies for diabetic symptoms. More research is needed to see what, if any, role these herbals may play. Check with your doctor before trying any herbal product.

What is unique about quinoa is not so much its protein content (3 grams in every 1/3-cup serving), but its protein composition. It contains all nine essential amino acids (amino acids the body cannot make), making it a complete protein. “With 13 grams of carbs per serving, it also contributes folate, magnesium, manganese, iron, and vitamin B6 to your diet. It has 2 grams of fiber, and it is very low in sodium,” Crawley says. Like other whole grain, high-fiber foods, quinoa can help prevent blood sugar spikes and stave off hunger.

Nausea and vomiting. When your body resorts to burning fat, it makes “ketones.” These can build up in your blood to dangerous levels, a possibly life-threatening condition called diabetic ketoacidosis. Ketones can make you feel sick to your stomach.

When diabetes occurs in women during pregnancy, it is called gestational diabetes. It usually is diagnosed between the 24th and 28th weeks of pregnancy. Like in type 1 and type 2 diabetes, blood sugar levels become too high. When women are pregnant, more glucose is needed to nourish the developing baby. The body needs more insulin, which is produced by the pancreas. In some women, the body does not produce enough insulin to meet this need, and blood sugar levels rise, resulting in gestational diabetes.

Additionally, most of my paternal relatives (my dad included), have, or have died from, diabetes. My personal experience with diabetes and subsequent review of the literature made it very clear to me that virtually every case of type 2 diabetes is reversible.

Every 21 seconds someone in the U.S. is diagnosed with diabetes. As of 2015, 30.3 million people — 9.4% of the population — have diabetes, with about 1.25 million American children and adults having type 1 diabetes. Shockingly, about 7.2 million people with diabetes are currently undiagnosed. Roughly 84 million people have prediabetes, when blood glucose levels are higher than normal but not yet high enough to be diagnosed as diabetes. Long-term damage to the heart and circulatory system can still occur with prediabetes. Diabetes is the seventh leading cause of death in the United States, as reported by the ADA.

“A 1/2-cup serving of cooked kale has only 18 calories and 4 grams of carbohydrate. It contains almost all the important nutrients, from vitamin A to zinc,” says Connie Crawley, RD, LD, Nutrition and Health Specialist at the University of Georgia Extension Service. “When you go to the farmer’s market, there are so many varieties to choose from, you are bound to find one that you like. It can be steamed, sauteed, microwaved, or stir-fried.”

A recent study found that people who kept a food diary lost twice as much weight as those who didn’t. Why? A written record helps you identify problem areas—such as your afternoon snack or your morning latte—where you’re getting more calories than you realized. It also increases your awareness of what, why, and how much you’re eating, which helps you cut back on mindless snacking.

People with T1D work with an endocrinologist to determine proper insulin-to-carb ratio. This ratio is the amount of insulin needed to balance the intake of a certain amount of carbohydrates (typically measured in grams). Measuring the amount of carbohydrates and factoring the insulin to carb (I:C) ratio helps maintain stable blood-sugar levels after eating.

The number one evidence-based method for blood sugar management is carbohydrate counting, or planning meals using carb “exchanges.” Every 15 grams of carbs are equal to 1 exchange, and you have a certain number of exchanges per meal. For example, if you’re aiming to eat 45 grams of carbs at lunch, you’ll budget out three exchanges. Carb counting was designed specifically for anyone taking insulin for diabetes management because it correlates directly with units of short-acting insulin.

It is thought that obese dogs and female dogs may run a greater risk of developing diabetes later in life (6-9 years of age). Some breeds may also run a greater risk, including Australian terriers, standard and miniature schnauzers, dachshunds, poodles, keeshonds and samoyeds. Juvenile diabetes can also be seen and is particularly prevalent in golden retrievers and keeshonds.

If you are overweight, combining physical activity with a reduced-calorie eating plan can lead to even more benefits. In the Look AHEAD: Action for Health in Diabetes study,1 overweight adults with type 2 diabetes who ate less and moved more had greater long-term health benefits compared to those who didn’t make these changes. These benefits included improved cholesterol levels, less sleep apnea, and being able to move around more easily.

The diabetes meal plan won’t tell you specific foods to eat, but it will guide you in selecting choices from the basic food groups and help you eat nutritious, balanced meals. Each meal and snack in the plan contains a certain of carbs, which works with the types and amount of insulin you take.

Garlic, the edible bulb from a plant in the lily family, has served as both a medicine and flavoring agent in cooking for thousands of years. It has been used to treat high cholesterol, heart disease, high blood pressure, and cancers. So does it really work? Here is what the current science says, according to the National Institutes of Health:

The goal of diabetes treatment is to keep blood glucose levels as close to normal as possible. Treatment for type 1 diabetes includes eating a healthy diet, exercising, and taking insulin. Treatment for type 2 includes living a healthy lifestyle and monitoring blood glucose levels. Self management is a key component for managing this condition, and includes meal planning, planned physical activity, and blood glucose monitoring.

We give you special kudos for managing your condition, as it is not always easy. If you’ve had diabetes for a long time, it’s normal to burn out sometimes. You may get tired of your day to day tasks, such as counting carbohydrates or measuring your blood sugar. Lean on a loved one or a friend for support, or consider talking to someone else who has diabetes who can provide, perhaps, an even more understanding ear or ideas that can help you.

People who have type 1 diabetes may also have nausea, vomiting, or stomach pains. Type 1 diabetes symptoms can develop in just a few weeks or months and can be severe. Type 1 diabetes usually starts when you’re a child, teen, or young adult but can happen at any age.

Also known as gliptins, DPP4 inhibitors have a number of effects, including stimulating pancreatic insulin (by preventing the breakdown of the hormone GLP-1). They may also help with weight loss through an effect on appetite.1-4

I do recommend this book solely out of the idea that it can help. But a strong word of caution. It has been my experience that what works for some, may not work for all. All the information herein is sound, medically. But it is ultimately the individual who is important and their own physical make-up. No how to can work for all. That is a given, but it can work for the vast majority.

Stay active. When you are active, your body uses sugar. You can use activity to help lower your blood sugar and manage your diabetes. Exercise also can help you lose weight and stay at a healthy weight.

Sulfonylureas, which also trigger insulin-releasing beta cells in your pancreas, are usually taken one or two times a day, before meals. Examples include glyburide, glimepiride, chlorpropamide, glipizide, and tolazamide.

Though it may be transient, untreated gestational diabetes can damage the health of the fetus or mother. Risks to the baby include macrosomia (high birth weight), congenital heart and central nervous system abnormalities, and skeletal muscle malformations. Second, can affect a person’s thinking process, coordination, and state of consciousness.[58][59] This disruption in brain functioning is called neuroglycopenia. Studies have demonstrated that the effects of neuroglycopenia impair driving ability.[58][60] A study involving people with type 1 diabetes found that individuals reporting two or more hypoglycemia-related driving mishaps differ physiologically and behaviorally from their counterparts who report no such mishaps.[61] For example, during hypoglycemia, drivers who had two or more mishaps reported fewer warning symptoms, their driving was more impaired, and their body released less epinephrine (a hormone that helps raise BG). Additionally, individuals with a history of hypoglycemia-related driving mishaps appear to use sugar at a faster rate[62] and are relatively slower at processing information.[63] These findings indicate that although anyone with type 1 diabetes may be at some risk of experiencing disruptive hypoglycemia while driving, there is a subgroup of type 1 drivers who are more vulnerable to such events.

Aside from the financial costs of diabetes, the more frightening findings are the complications and co-existing conditions. In 2014, 7.2 million hospital discharges were reported with diabetes as a listed diagnosis. Patients with diabetes were treated for major cardiovascular diseases, ischemic heart disease, stroke, lower-extremity amputation and diabetic ketoacidosis.

The total daily calories are evenly divided into three meals (with snacks for youth with type 1 diabetes). Over the past two years the ADA has lifted the absolute ban on simple sugars for people with diabetes. Small amounts of simple sugars are now allowed when consumed with a complex meal.

According to About.com, a half-cup serving of firm tofu contains 10 grams of protein, 5 grams of fat, and only 2 grams of carbohydrates. In addition to tofu, a number of soy products like tempeh and miso contain a lot of protein and few carbs.

Meanwhile, saturated fats and trans fats can harm your heart and overall health, according to the American Heart Association. To spot trans fats, look for the term “hydrogenated” on labels of processed foods, such as packaged snacks, baked goods, and crackers. “I always tell my clients to double-check the ingredient list to make sure they don’t see any partially hydrogenated oil in their food products,” Massey says.

Regular blood testing, especially in type 1 diabetics, is helpful to keep adequate control of glucose levels and to reduce the chance of long term side effects of the disease. There are many (at least 20+) different types of blood monitoring devices available on the market today; not every meter suits all patients and it is a specific matter of choice for the patient, in consultation with a physician or other experienced professional, to find a meter that they personally find comfortable to use. The principle of the devices is virtually the same: a small blood sample is collected and measured. In one type of meter, the electrochemical, a small blood sample is produced by the patient using a lancet (a sterile pointed needle). The blood droplet is usually collected at the bottom of a test strip, while the other end is inserted in the glucose meter. This test strip contains various chemicals so that when the blood is applied, a small electrical charge is created between two contacts. This charge will vary depending on the glucose levels within the blood. In older glucose meters, the drop of blood is placed on top of a strip. A chemical reaction occurs and the strip changes color. The meter then measures the color of the strip optically.

Even with careful management, blood sugar levels can sometimes change unpredictably. With help from your diabetes treatment team, you’ll learn how your blood sugar level changes in response to food, physical activity, medications, illness, alcohol, stress — for women, fluctuations in hormone levels.

Diabetes is a chronic condition associated with abnormally high levels of sugar (glucose) in the blood. Insulin produced by the pancreas lowers blood glucose. Absence or insufficient production of insulin, or an inability of the body to properly use insulin causes diabetes.

In patients with metformin contraindications or intolerance, consider an initial drug from another class depicted in Fig. 8.1 under “Dual Therapy” and proceed accordingly. When A1C is ≥9% (75 mmol/mol), consider initiating dual combination therapy (Fig. 8.1) to more expeditiously achieve the target A1C level. Insulin has the advantage of being effective where other agents may not be and should be considered as part of any combination regimen when hyperglycemia is severe, especially if catabolic features (weight loss, ketosis) are present. Consider initiating combination insulin injectable therapy (Fig. 8.2) when blood glucose is ≥300 mg/dL (16.7 mmol/L) or A1C is ≥10% (86 mmol/mol) or if the patient has symptoms of hyperglycemia (i.e., polyuria or polydipsia). As the patient’s glucose toxicity resolves, the regimen may, potentially, be simplified.

For people who take insulin or another blood glucose-lowering medication that can cause hypoglycemia, eat some food when drinking alcohol. A cautionary note to people with type 1 diabetes: Significant alcohol intake can cause hypoglycemia a number of hours later, so regularly check blood sugar levels when you drink.

“For people with diabetes who don’t respond to oral medications or non-insulin injectables, insulin can be started as a long-acting preparation once a day,” Sivitz says. Short-acting insulin may be added before meals if long-acting insulin alone isn’t effective enough.

Fig. 8.2 outlines these options, as well as recommendations for further intensification, if needed, to achieve glycemic goals. If a patient is still above the A1C target on premixed insulin twice daily, consider switching to premixed analog insulin three times daily (70/30 aspart mix, 75/25 or 50/50 lispro mix). In general, three times daily premixed analog insulins have been found to be noninferior to basal-bolus regimens with similar rates of hypoglycemia (62). If a patient is still above the A1C target on basal insulin plus single injection of rapid-acting insulin before the largest meal, advance to a basal-bolus regimen with ≥2 injections of rapid-acting insulin before meals. Consider switching patients from one regimen to another (i.e., premixed analog insulin three times daily to basal-bolus regimen or vice-versa) if A1C targets are not being met and/or depending on other patient considerations (60,61). Metformin should be continued in patients on combination injectable insulin therapy, if not contraindicated and if tolerated, for further glycemic benefits.

Because the symptoms of low blood sugar alert you to the problem, not having any symptoms requires that you be especially vigilant. Remember: Frequent monitoring is the only way to know if you are low and need to take corrective action.

SI: That was my moment of truth, if you will. I thought, it doesn’t make any sense. What we are telling people to do, the message to pre-diabetics today in America is “Don’t you worry. You’re getting fit but you have to eat a little less, eat a little healthier and exercise a little more everything will be just fine.” Well I had been doing that freaking 20 years.

Some people may be able to control their type 2 diabetes symptoms by losing weight, following a healthy diet, doing plenty of exercise, and monitoring their blood glucose levels. However, type 2 diabetes is typically a progressive disease – it gradually gets worse – and the patient will probably end up have to take insulin, usually in tablet form.

PCOS is a metabolic syndrome caused due to hormonal imbalance in the female body. It affects fertility in women and causes irregular periods. Other symptoms include facial hair, loss of hair, acne, weight gain, skin darkening (especially around the neck, elbows and armpit) and depression. Females with PCOS are at a great risk of getting diabetes. PCOS is thus an important warning sign of an impending diabetic condition.

SI: Yeah, that’s precisely the reason why we have our own doctors. That’s precisely the reason why we have a full-stack health care company…It’s absolutely critical that we get this data very, very rapidly so we can start adjusting them off of meds. If we don’t get people off the meds quickly and accurately, it’s actually dangerous for the patients. When you’re diabetic your blood sugar runs high. You take drugs, you get it down. If we can get your blood sugar down naturally and then you take drugs on top of that you’re going to be in the ER.

Work with your doctor. Some risks related to diabetes, such as heart disease, are still higher than normal even when your blood sugar is controlled. So work closely with your doctor, and go to all of your appointments.

You’re likely to start by seeing your primary care doctor if you’re having diabetes symptoms. If your child is having diabetes symptoms, you might see your child’s pediatrician. If blood sugar levels are extremely high, you’ll likely be sent to the emergency room.

Type 2 diabetes is typically diagnosed during adulthood, usually after age 45 years. It was once called adult-onset diabetes mellitus, or non-insulin-dependent diabetes mellitus. These names are no longer used because type 2 diabetes does occur in young people, and some people with type 2 diabetes require insulin therapy.

Insulin is not available in oral tablet form, although a new inhaled insulin product called Afrezza was approved in 2014. Afrezza is an ultra rapid-acting inhaled that is administered with meals to improve blood sugar control in adult diabetics. Insulin is usually is delivered by injections that are required one to four times per day. Some people use an insulin pump, which is worn at all times and delivers a steady flow of insulin throughout the day.

Glycated hemoglobin (A1C) test. This blood test indicates your average blood sugar level for the past two to three months. It measures the percentage of blood sugar attached to hemoglobin, the oxygen-carrying protein in red blood cells. The higher your blood sugar levels, the more hemoglobin you’ll have with sugar attached. An A1C level of 6.5 percent or higher on two separate tests indicates that you have diabetes. An A1C between 5.7 and 6.4 percent indicates prediabetes. Below 5.7 is considered normal.

To prevent further diabetic complications as well as serious oral problems, diabetic persons must keep their blood sugar levels under control and have a proper oral hygiene. A study in the Journal of Periodontology found that poorly controlled type 2 diabetic patients are more likely to develop periodontal disease than well-controlled diabetics are.[71] At the same time, diabetic patients are recommended to have regular checkups with a dental care provider at least once in three to four months. Diabetics who receive good dental care and have good insulin control typically have a better chance at avoiding gum disease to help prevent tooth loss.[74]

The earliest surviving work with a detailed reference to diabetes is that of Aretaeus of Cappadocia (2nd or early 3rd century CE). He described the symptoms and the course of the disease, which he attributed to the moisture and coldness, reflecting the beliefs of the “Pneumatic School”. He hypothesized a correlation of diabetes with other diseases, and he discussed differential diagnosis from the snakebite which also provokes excessive thirst. His work remained unknown in the West until 1552, when the first Latin edition was published in Venice.[101]

Depending on your treatment plan, you may need to check and record your blood sugar level every now and then or, if you’re on insulin, multiple times a day. Ask your doctor how often he or she wants you to check your blood sugar. Careful monitoring is the only way to make sure that your blood sugar level remains within your target range.

Many pregnant women develop insulin resistance and high blood sugar levels during pregnancy. This is known as gestational diabetes. Gestational diabetes must be monitored by a woman’s obstetrician throughout her pregnancy, as it can lead to complications for mother and baby. Gestational diabetes usually goes away after the woman gives birth.

Type 2 diabetes is the leading cause of diabetes-related complications, such as blindness, non-traumatic amputations and chronic kidney failure. In fact, diabetes is the leading cause of kidney disease, and it’s called diabetic kidney disease. It also raises the risk for heart disease, stroke and reproductive/fertility problems.

Oftentimes, your symptoms may seem harmless. The most common symptoms of diabetes, such as persistent thirst and fatigue, are often vague. When experienced on their own, symptoms such as these may not be anything to worry about.

The Dietary Guidelines for Americans recommend that everyone, including people with diabetes, eat 2-1/2 cups of vegetables per day. If your blood sugar rises after you eat vegetables, check your portion sizes:

Once you’ve had your insulin injection, you can’t stop the medicine from working. If you take an insulin shot but forget to eat, your blood sugar levels can get too low. So try to avoid skipping meals or snacks. If your parents remind you to eat when you take your insulin, it’s probably because they worry about you, not because they’re trying to nag you!

Since carbohydrate is the macronutrient that raises blood glucose levels most significantly, the greatest debate is how low in carbohydrates the diet should be. This is because although lowering carbohydrate intake will help reduce blood glucose levels, a low-carbohydrate diet conflicts with the traditional establishment view that carbohydrates should be the main source of calories. Recommendations of the fraction of total calories to be obtained from carbohydrate are generally in the range of 20% to 45%,[1] but recommendations can vary as widely as from 16% to 75%.[2]

Like pramlintide, exenatide is injected but only twice each day (usually before breakfast and dinner meals). Exenatide is available by a disposable pen form and in two doses. Similar to pramlintide, the main side effect of exenatide is nausea, most likely due to its effects on stomach emptying. Exenatide is temperature sensitive, so the initial recommendation was to store pens at 36 F to 46 F (2 C to 8 C). This recommendation recently changed. Unopened pens should be refrigerated; once opened, exenatide pens can be left at room temperature. The risk of hypoglycemia remains a possibility with exenatide, especially when used in combination with sulfonylurea. Your health-care professional may choose to decrease the dose of other medications while initially evaluating your response to exenatide.

Symptoms of type 1 diabetes can start quickly, in a matter of weeks. Symptoms of type 2 diabetes often develop slowly—over the course of several years—and can be so mild that you might not even notice them. Many people with type 2 diabetes have no symptoms. Some people do not find out they have the disease until they have diabetes-related health problems, such as blurred vision or heart trouble.

Endocrinology is the specialty of medicine that deals with hormone disturbances, and both endocrinologists and pediatric endocrinologists manage patients with diabetes. People with diabetes may also be treated by family medicine or internal medicine specialists. When complications arise, people with diabetes may be treated by other specialists, including neurologists, gastroenterologists, ophthalmologists, surgeons, cardiologists, or others.

Diabetes is a metabolic disorder that occurs when your blood sugar (glucose), is too high (hyperglycemia). Glucose is what the body uses for energy, and the pancreas produces a hormone called insulin that helps convert the glucose from the food you eat into energy. When the body either does not produce enough insulin, does not produce any at all, or your body becomes resistant to the insulin, the glucose does not reach your cells to be used for energy. This results in the health condition termed diabetes.

Your health care team also can help you decide the best time of day for you to do physical activity based on your daily schedule, meal plan, and diabetes medicines. If you take insulin, you need to balance the activity that you do with your insulin doses and meals so you don’t get low blood glucose.

Especially avoid gluten-containing grains and white wheat flour products such as bread, bagels, pretzels, cereals and crackers.  All grains break down into sugars and have the potential to severely spike blood glucose. Because physical activity lowers your blood glucose, you should protect yourself against low blood glucose levels, also called hypoglycemia. You are most likely to have hypoglycemia if you take insulin or certain other diabetes medicines, such as a sulfonylurea. Hypoglycemia also can occur after a long intense workout or if you have skipped a meal before being active. Hypoglycemia can happen during or up to 24 hours after physical activity.

It seems clear that the successful ones eat very low amounts of refined sugars and simple starches. They may have small amounts of truly whole grains (not stuff that is marketed as “whole grain” but is actually highly processed). They eat small amounts of fruits and starchy vegetables. Given the above research findings, it is recommended that drivers with type 1 diabetes with a history of driving mishaps should never drive when their BG is less than 70 mg/dl (3.9 mmol/l). Instead, these drivers are advised to treat hypoglycemia and delay driving until their BG is above 90 mg/dl (5 mmol/l).[61] Such drivers should also learn as much as possible about what causes their hypoglycemia, and use this information to avoid future hypoglycemia while driving.

If, on the other hand, you are already starting to develop complications or your medication regimen has changed because your blood sugars are getting higher, remember that diabetes is a progressive disease—and sometimes these things just happen without any influence from your own actions. As you age, beta cells in the pancreas get tired and stop working. If you’ve had diabetes for 20 years and now need to start insulin, for example, it doesn’t mean you’ve failed. It just means that your body needs some help. Make sure you continue to receive education and that you continue to have someone to lean on when you need it, and keep the lines of communication open with your doctor. Barriers to adherence have been associated with key psychological mechanisms: knowledge of self-management, beliefs about the efficacy of treatment and self-efficacy/perceived control.[78] Such mechanisms are inter-related, as one’s thoughts (e.g. one’s perception of diabetes, or one’s appraisal of how helpful self-management is) is likely to relate to one’s emotions (e.g. motivation to change), which in turn, affects one’s self-efficacy (one’s confidence in their ability to engage in a behaviour to achieve a desired outcome).[79]

If I eat sweets it is a bite or two..unless I give myself a splurge moment. Can do without spiking. I agree that vegs and protein, plus sprouted grain slice of bread once a day works well. I eat quite a lot of fruit by itself, seems to aid the whole process.

Since it is a dietary disorder, it figures that what you eat would play a huge role in this reversal. Yes, calories are important to the extent that if you burn up far less than what you consume, you will put on weight and worsen insulin resistance. But to imagine a slice of bread and an egg are the same just because they contain the same number of calories is one of those diabetes myths that has run out of favor with almost every medical expert. The slice of bread will certainly leave you far worse with your diabetes than the egg will because of how these two foods behave inside your body.

Being smart about sweets is only part of the battle. Sugar is also hidden in many packaged foods, fast food meals, and grocery store staples such as bread, cereals, canned goods, pasta sauce, margarine, instant mashed potatoes, frozen dinners, low-fat meals, and ketchup. The first step is to spot hidden sugar on food labels, which can take some sleuthing:

Amylin synthetic derivatives: Amylin is a naturally occurring hormone secreted by the pancreas along with insulin. An amylin derivative, such as pramlintide (Symlin), can help control blood sugar when insulin alone does not. Pramlintide is administered as a subcutaneous injection along with insulin and helps achieve lower blood sugar levels after meals, helps reduce fluctuation of blood sugar levels throughout the day, and improves hemoglobin A1c levels.

Glucose is the main source of energy for the body, and it comes from carbohydrate in our foods. When blood glucose levels rise, the hormone insulin is released from the pancreas into the bloodstream. This helps the body use glucose effectively.

Even small amounts of physical activity can help. Experts suggest that you aim for at least 30 minutes of moderate or vigorous physical activity 5 days of the week.3 Moderate activity feels somewhat hard, and vigorous activity is intense and feels hard. If you want to lose weight or maintain weight loss, you may need to do 60 minutes or more of physical activity 5 days of the week.3

Trick (not as important): Exercise more. Regularly giving your muscles a workout has been shown to increase insulin sensitivity. I say exercise “more” because if you’re not getting any exercise at all, you’re not even trying and probably haven’t read this far anyway.

While at least certain diabetes mellitus symptoms usually become obvious after some time, some people with type 2 diabetes have symptoms so mild that they go totally unnoticed. This is especially true among women with gestational diabetes, the type that develops during pregnancy and usually only lasts for a short period of time. Women with gestational diabetes often have no noticeable symptoms at all, which is why it’s important for at-risk women to be tested and monitored in order to prevent complications and ensure a healthy, vibrant pregnancy. The Diabetes Prevention Program conducted a randomized clinical trial over three years and found that diabetes incidence in high-risk adults was reduced by 58 percent after they followed intensive lifestyle intervention compared to 31 percent after taking medication (metformin). Aside from the financial costs of diabetes, the more frightening findings are the complications and co-existing conditions. In 2014, 7.2 million hospital discharges were reported with as a listed diagnosis. Patients with diabetes were treated for major cardiovascular diseases, ischemic heart disease, stroke, lower-extremity amputation and diabetic ketoacidosis.

PCOS is a metabolic syndrome caused due to hormonal imbalance in the female body. It affects fertility in women and causes irregular periods. Other symptoms include facial hair, loss of hair, acne, weight gain, skin darkening (especially around the neck, elbows and armpit) and depression. Females with PCOS are at a great risk of getting diabetes. PCOS is thus an important warning sign of an impending diabetic condition.

Adhering to the following guidelines can help you do at least three things that are essential for successfully treating diabetes: recover your insulin/leptin sensitivity; normalize your weight; and normalize your blood pressure.

Rating your plate is a meal planning system based upon portion size. Imaginary lines are used to divide a meal plate into two halves, and one half is further divided into fourths. One-fourth of the plate should contain grains/starches, one-fourth should contain protein, and the remaining half should contain non-starchy vegetables.

By checking your own blood sugar levels, you can track your body’s changing needs for insulin and work with your doctor to figure out the best insulin dosage. People with diabetes check their blood sugar up to several times a day with an instrument called a glucometer. The glucometer measures glucose levels in a sample of your blood dabbed on a strip of treated paper. Also, there are now devices, called continuous glucose monitoring systems (CGMS), that can be attached to your body to measure your blood sugars every few minutes for up to a week at a time. But these machines check glucose levels from skin rather than blood, and they are less accurate than a traditional glucometer.

The risk of developing type 2 diabetes increases with age, and the disease becomes more common in people over age 45. As stated above, anyone 45 years of age or older who is overweight or obese and that has at least one other risk factor for diabetes should get tested for diabetes.

Type 2 DM begins with insulin resistance, a condition in which cells fail to respond to insulin properly.[2] As the disease progresses a lack of insulin may also develop.[9] This form was previously referred to as “non insulin-dependent diabetes mellitus” (NIDDM) or “adult-onset diabetes”.[2] The most common cause is excessive body weight and insufficient exercise.[2]

Weight loss surgery in those with obesity and type two diabetes is often an effective measure.[85] Many are able to maintain normal blood sugar levels with little or no medications following surgery[86] and long-term mortality is decreased.[87] There is, however, a short-term mortality risk of less than 1% from the surgery.[88] The body mass index cutoffs for when surgery is appropriate are not yet clear.[87] It is recommended that this option be considered in those who are unable to get both their weight and blood sugar under control.[89]

Optimize your vitamin D levels. Recent studies have revealed that getting enough vitamin D can have a powerful effect on normalizing your blood pressure and that low vitamin D levels may increase your risk of heart disease.

Subsequent gestational diabetes. Once you’ve had gestational diabetes in one pregnancy, you’re more likely to have it again with the next pregnancy. You’re also more likely to develop diabetes — typically type 2 diabetes — as you get older.

The American Diabetes Association and many experts recommend that 50% to 60% of daily calories come from carbohydrates, 12% to 20% from protein, and no more than 30% from fat. People with diabetes may benefit from eating small meals throughout the day, instead of eating one or two heavy meals. No foods are absolutely forbidden for people with diabetes. Attention to portion control and advance meal planning can help people with diabetes enjoy the same meals as everyone else.

“People with diabetes often describe being very thirsty,” Silver says. When your blood sugar levels are high, the amount of sodium in your blood tends to drop. Sodium helps your body hold onto water, so a drop in blood sodium can increase thirst, research shows. Drinking water will resolve the issue temporarily. But if you feel like you’re much thirstier than you used to be (even though you’re drinking the same amount of water) that’s warning sign.

These drugs rapidly lower blood sugar, but can cause abnormally low blood sugar (called hypoglycemia). In addition, sulfonylureas contain sulfa and should be avoided by patients who are allergic to sulfa.

Type 2 diabetes is typically diagnosed during adulthood, usually after age 45 years. It was once called adult-onset diabetes mellitus, or non-insulin-dependent diabetes mellitus. People with insulin resistance develop type 2 diabetes when they fail to secrete enough insulin to cope with their body’s demands.

Diet is an important tool to keep your heart healthy and blood sugar levels within a safe and healthy range. It doesn’t have to be complicated or unpleasant. The diet recommended for people with type 2 diabetes is the same diet just about everyone should follow. It boils down to a few key actions:

A person with diabetes has a condition in which the quantity of glucose in the blood is too elevated (hyperglycemia). This is because the body either does not produce enough insulin, produces no insulin, or has cells that do not respond properly to the insulin the pancreas produces. This results in too much glucose building up in the blood. This excess blood glucose eventually passes out of the body in urine. So, even though the blood has plenty of glucose, the cells are not getting it for their essential energy and growth requirements.

Your doctor will check your blood glucose levels, and if you are diagnosed with diabetes, your doctor will guide you on a plan to keep your blood sugar levels normal. If your diabetes is mild, your doctor will likely recommend a diet plan, exercise, and weight loss. Your doctor may prescribe medications that help reduce blood sugar levels. In some women, insulin may be necessary.

Exenatide is the first in the incretin mimetic class of drugs for type 2 diabetes. Exenatide shares many therapeutic properties with GLP-1, and it mimics natural physiology for self-regulating blood sugar. Namely, exenatide slows stomach emptying and slows the release of glucose from the liver, thereby regulating delivery of nutrients to the intestine for absorption. Exenatide also works centrally in the brain to regulate hunger.

Note: Type 1 diabetes must be treated with insulin; if you have type 2 diabetes, you may not need to take insulin. This involves injecting insulin under the skin for it to work. Insulin cannot be taken as a pill because the digestive juices in the stomach would destroy the insulin before it could work. Scientists are looking for new ways to give insulin. But today, shots are the only method. There are, however, new methods to give the shots. Insulin pumps are now being widely used and many people are having great results.

Other forms of diabetes mellitus include congenital diabetes, which is due to genetic defects of insulin secretion, cystic fibrosis-related diabetes, steroid diabetes induced by high doses of glucocorticoids, and several forms of monogenic diabetes.

Equipping patients with an algorithm for self-titration of insulin doses based on self-monitoring of blood glucose improves glycemic control in patients with type 2 diabetes initiating insulin (42). Comprehensive education regarding self-monitoring of blood glucose, diet, and the avoidance of and appropriate treatment of hypoglycemia are critically important in any patient using insulin.
