I find that if I eat carbs at breakfast or during the day I utilize them better. In the evening, I eat just protein and vegetables. I make a home made cheese cake that has very few carbs because I use stevia and thus have a dessert. Once in a while I eat one square of 85 percent chocolate. I can eat a baked potato about once a month, and rice never as even the smallest amount of rice, no matter what kind of grain it is, will spike my blood sugar as if it were bread, of which I can eat very little.

In contrast, other GLP-1 receptor agonists have not shown similar reductions in cardiovascular events (Table 9.4). Whether the benefits of GLP-1 receptor agonists are a class effect remains to be definitively established. A randomized clinical trial similarly found that, among overweight adolescents with type 1 diabetes, the addition of metformin to insulin did not improve glycemic control and increased risk for gastrointestinal adverse events after 6 months compared with placebo (23). The Reducing With Metformin Vascular Adverse Lesions in Type 1 Diabetes (REMOVAL) trial investigated the addition of metformin therapy to titrated insulin therapy in adults with type 1 diabetes at increased risk for cardiovascular disease and found that metformin did not significantly improve glycemic control beyond the first 3 months of treatment and that progression of atherosclerosis (measured by carotid artery intima-media thickness) was not significantly reduced, although other cardiovascular risk factors such as body weight and LDL cholesterol improved (24). Metformin is not FDA-approved for use in patients with type 1 diabetes. A random blood sugar test may reveal high blood sugar levels. A hemoglobin A1C test can provide more information about average blood sugar levels over a few months. Your child may also need a fasting blood sugar test. The name of the alpha-glucosidase inhibitor available in the U.S. is acarbose (Precose). Placebo-controlled clinical trials with over 700 patients associated acarbose with reduction of hemoglobin A1c (HbA1c) values. HbA1c is the standard clinical measure of average blood sugars over the preceding three months. However, as a single agent, acarbose has not been as effective as other antidiabetic medications. Since acarbose works in the intestine, its effects are additive to antidiabetic medications that work at other sites (such as sulfonylureas). Clinical studies have demonstrated statistically better control of blood glucose in patients treated with acarbose and a sulfonylurea, compared to the sulfonylurea alone. Acarbose is currently used alone or in combination with a sulfonylurea. Second, hypoglycemia can affect a person’s thinking process, coordination, and state of consciousness.[58][59] This disruption in brain functioning is called neuroglycopenia. Studies have demonstrated that the effects of neuroglycopenia impair driving ability.[58][60] A study involving people with type 1 diabetes found that individuals reporting two or more hypoglycemia-related driving mishaps differ physiologically and behaviorally from their counterparts who report no such mishaps.[61] For example, during hypoglycemia, drivers who had two or more mishaps reported fewer warning symptoms, their driving was more impaired, and their body released less epinephrine (a hormone that helps raise BG). For people with either type of diabetes, exercise can lower the chance of having a heart attack or stroke and can improve circulation. It may offer stress relief, as well. People with type 2 diabetes who need to lose weight can benefit from moderate exercise. Most people with diabetes are encouraged to get at least 150 minutes each week of moderate-intensity aerobic physical activity, like walking. Type 2 diabetes is the most common form of diabetes, and unlike type 1 diabetes, it usually occurs in people over the age of 40, especially those who are overweight. Type 2 diabetes is caused by insulin resistance, which means that the hormone insulin is being released, but a person doesn’t respond to it appropriately. Type 2 diabetes is a metabolic disorder that’s caused by high blood sugar. The body can keep up for a period of time by producing more insulin, but over time the insulin receptor sites burn out. Eventually, diabetes can affect nearly every system in the body, impacting your energy, digestion, weight, sleep, vision and more. (5)

In the United States alone, more than 8 million people have undiagnosed diabetes, according to the American Diabetes Association. But you don’t need to become a statistic. Understanding possible diabetes symptoms can lead to early diagnosis and treatment — and a lifetime of better health. According to the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK), you can calculate the amount of carbs you need by first figuring out what percentage of your diet should be made up of carbohydrates. (The NIDDK notes that experts generally recommend this number be somewhere between 45 and 65 percent of your total calories, but people with diabetes are almost always recommended to stay lower than this range.) Multiply that percentage by your calorie target. For example, if you’re aiming to get 50 percent of your calories from carbs and you eat 2,000 calories a day, you’re aiming for about 1,000 calories of carbs. Because the NIDDK says 1 gram (g) of carbohydrates provides 4 calories, you can divide the calories of carbs number by 4 to get your daily target for grams of carbs, which comes out to 250 g in this example. For a more personalized daily carbohydrate goal, it’s best to work with a certified diabetes educator or a registered dietitian to determine a goal that is best for you.

It is important to realize that sugar is not the only carbohydrate that you have to “control.” The body will convert all carbohydrates to glucose, so eating extra servings of rice, pasta, bread, fruit, or other carbohydrate foods will make the blood sugar rise.

An increasing number of top medical doctors, nutritionists and scientists believe that through a simple diabetic diet, type-2 diabetes can become an entirely reversible dietary disorder. Jason Fung, M.D. says “Once you get the diagnosis, it’s a sentence. But, it’s actually a great big lie. Type 2 diabetes is almost always reversible and this is almost ridiculously easy to prove.”

For those who are taking the baby-steps approach to eating better, this list is even more helpful. Hyperglycemic hyperosmolar nonketotic syndrome (HHNS). Signs and symptoms of this life-threatening condition include a blood sugar reading higher than 600 mg/dL (33.3 mmol/L), dry mouth, extreme thirst, fever greater than 101 F (38 C), drowsiness, confusion, vision loss, hallucinations and dark urine. Your blood sugar monitor may not be able to give you an exact reading at such high levels and may instead just read “high.”

More modern history of the diabetic diet may begin with Frederick Madison Allen and Elliott Joslin, who, in the early 20th century, before insulin was discovered, recommended that people with diabetes eat only a low-calorie and nearly zero-carbohydrate diet to prevent ketoacidosis from killing them. While this approach could extend life by a limited period, patients developed a variety of other medical problems.[7]

Pramlintide, an amylin analog, is an agent that delays gastric emptying, blunts pancreatic secretion of glucagon, and enhances satiety. It is FDA-approved for use in adults with type 1 diabetes. It has been shown to induce weight loss and lower insulin doses. Concurrent reduction of prandial insulin dosing is required to reduce the risk of severe hypoglycemia.

Regardless of the form, soy products have a deserved reputation for providing high-quality protein that is low in saturated fat and cholesterol. In fact, soy is a great way to help meet the 2010 Dietary Guidelines for Americans recommendation to “replace protein foods that are high in solids fats (such as many meats) with choices that are lower in solid fats and calories.”

Physical activity is an important part of controlling diabetes and preventing complications such as heart disease and high blood pressure. “We know that exercise is a very effective way to help bring blood sugars under control for someone with type 2 diabetes,” says Kenneth Snow, M.D., Acting Chief, Adult Diabetes, Joslin Clinic. Try for 30 minutes of moderate exercise, like brisk walking, on most days. Joslin’s Why WAIT? and Easy Start exercise programs are great resources for developing a safe weight loss program.

It is very important to recognize hypoglycemia and be prepared to treat it at all times. Headache, feeling dizzy, poor concentration, tremor of the hands, and sweating are common symptoms of hypoglycemia. While it may sound intimidating for someone who’s just been diagnosed, the simplest thing you can do right away is to stop skipping meals. Going for long periods without eating isn’t the best idea for anyone. It can slow your metabolism over time, make it easier to overindulge later and mess with your energy levels. But for anyone with diabetes or prediabetes, it’s even more significant. Meal skipping can lead to dips and spikes in blood sugar, so eating once every three to four hours is a good goal to set.

Although sugar does not cause the blood sugar to rise any higher than other carbohydrates, it should be eaten along with other healthy foods. If you choose to drink a 12-ounce can of a sugar-sweetened soft drink, that would use up about 45 grams of carba, and you wouldn’t have gotten any nutrition (protein, vitamins, or minerals). What a waste of calories!

Regular exercise is especially important for people with diabetes. It helps with blood sugar control, weight loss, and high blood pressure. People with diabetes who exercise are less likely to experience a heart attack or stroke than diabetics who do not exercise regularly. You should be evaluated by your physician before starting an exercise program.

Strength training is a light or moderate physical activity that builds muscle and helps keep bones healthy. Strength training is important for both men and women. When you have more muscle and less body fat, you’ll burn more calories. Burning more calories can help you lose and keep off extra weight.

Cooked or raw, carrots are a healthy addition to any meal plan. While cooked carrots have the rich texture of starchy vegetables, such as potatoes, they are classified as nonstarchy veggies because they don’t contain a lot of carbohydrate. A 1-cup serving of raw carrots has about 5 grams of carb, as does a 1/2-cup cooked serving. According to the American Diabetes Association, five baby carrots are considered a “free food” and do not need to be counted in a meal plan.

It’s estimated by the American Diabetes Association that 30.3 million Americans have one of three forms of diabetes (type 1, type 2 or gestational). This equals about 9.4 percent of the population or about one in every 11 people. (10a)

Because the insulin hormone isn’t getting glucose into the cells, where it can be used as energy, the body thinks it’s starving and starts breaking down protein from the muscles as an alternate source of fuel.

Because normal digestion interferes with insulin taken by mouth, insulin must be injected. Depending on your needs, your doctor may prescribe a mixture of insulin types to use throughout the day and night. Often, people with type 2 diabetes start insulin use with one long-acting shot at night.

Diabetic ketoacidosis can be caused by infections, stress, or trauma, all of which may increase insulin requirements. In addition, missing doses of insulin is also an obvious risk factor for developing diabetic ketoacidosis. Urgent treatment of diabetic ketoacidosis involves the intravenous administration of fluid, electrolytes, and insulin, usually in a hospital intensive care unit. Dehydration can be very severe, and it is not unusual to need to replace 6-7 liters of fluid when a person presents in diabetic ketoacidosis. Antibiotics are given for infections. With treatment, abnormal blood sugar levels, ketone production, acidosis, and dehydration can be reversed rapidly, and patients can recover remarkably well.

Manufacturers are required to provide the total amount of sugar in a serving but do not have to spell out how much of this sugar has been added and how much is naturally in the food. The trick is deciphering which ingredients are added sugars. Aside from the obvious ones—sugar, honey, molasses—added sugar can appear as agave nectar, cane crystals, corn sweetener, crystalline fructose, dextrose, evaporated cane juice, fructose, high-fructose corn syrup, invert sugar, lactose, maltose, malt syrup, and more.
